KINDERGARTEN REGISTRATION
PLEASE PROVIDE THE FOLLOWING:
Proof of residency: Two current utility bills (i.e. water, electric, gas) or Driver License with correct address and (1) current
utility bill, or if you are moving into the area you MUST have your escrow papers which should have your actual address on
them along with your signature.
Original Legal Birth Certificate (not hospital certificate), Baptismal record or Passport.
Kindergarten - Must be born on or before Dec. 2, 2004
Report of Health Examination for School Entry (Grades Kdg. and 1st only) - this form must be completed by a physician and
Portola Elementary School office prior to the start of school.
Official Immunization record signed by a doctor's office or clinic.
Dental Exam due by May, 2010 (Kindergarten only)
Student's Social Security Number.
IMMUNIZATION REQUIREMENTS FOR KINDERGARTEN
Polio: 4 doses, but 3 doses are enough if at least one was given after the 4th birthday
DPT/ DT: 4 or more doses, with last dose after 4th birthday
Measles, Mumps, Rubella: 2 doses of measles-containing vaccine
Hepatitis B: 3 doses (children who have received at least one dose of Hepatitis B vaccine may enter
Kindergarten on the condition that they receive the remaining doses within appropriate
Time intervals.
Varicella: 1 dose OR a signed statement from the doctor stating that the child has had the Chickenpox disease
